Appointment Confirmation

Lifted Minds requests appointment confirmation at least 48 hours in advance. If your appointment remains unconfirmed, our office will make follow-up outreach. Within 24 hours of the scheduled visit, an unconfirmed appointment may be released after clinical review and notice, consistent with applicable payer requirements and continuity-of-care obligations.

This is a Lifted Minds scheduling policy. Lack of confirmation alone is not a patient-initiated cancellation or no-show. No late-cancellation or no-show fee applies when the practice releases an appointment for nonconfirmation.

Late Cancellations & No-Shows

A $75 fee may apply to qualifying patient-initiated cancellations with less than 24 hours’ notice or missed appointments, where permitted by your payer and disclosed in advance. Insurance does not cover cancellation fees. Headway-billed cancellation fees are processed through Headway under its applicable rules.

Payment & Outstanding Balances

A valid payment method is required through the applicable secure billing portal. Outstanding balances must be addressed or an approved payment arrangement established, consistent with applicable payer requirements. Financial issues affecting scheduling are reviewed individually with attention to clinical needs and continuity of care.

For care billed through Headway, Headway handles billing and collection. Contact Headway directly about its balances, payment options, or billing disputes. A Lifted Minds payment arrangement does not settle or change a Headway balance or insurance-required cost-sharing.

Self-Pay Eligibility

Self-pay availability depends on your coverage and applicable agreements. For services billed through Headway, self-pay is not available when you have insurance with which Headway is in-network. Other Headway eligibility restrictions may apply; our office will confirm eligibility before scheduling. These prices do not change insurance billing rates.

Financial Hardship & Payment Assistance

Please contact us before your appointment if cost is a concern. Reduced self-pay fees or payment arrangements may be considered individually for eligible services; availability is not guaranteed. Insurance cost-sharing and Headway payment assistance remain subject to the applicable payer and Headway requirements.
